Insurance Policy Contacts

We have received a request regarding contacts for the Chubb and Hartford insurance policies. The information we have is posted below.

PLEASE NOTE: The Chubb Policy expires on November 30, 2006. If you are going to file a claim against this policy, please do so quickly.

HOW TO FILE A
PROOF OF CLAIM

1. Download the B10 Proof of Claim form (above).

2. The claim form MUST be filled out completely and MUST include any attachments needed to support your claim. DO NOT SUBMIT ORIGINAL backup up documentation to support your claim; it will not be returned to you.

3. The original claim must have your signature on it (not a stamp or copy), preferably in blue ink. You must return the original of your completed claim, with all attachments, to the Clerk's Office (U.S. Bankruptcy Court, Attention: Court Clerk, 701 Broadway, Nashville, TN 37203).

4. If you wish to have a confirmed copy of your claim returned to you, please bring an extra for yourself, or if you are mailing it, include an extra copy with a self addressed, stamped envelope with enough postage to cover return mailing costs. The Clerk will file stamp the extra copy and return it to you.

5. IF YOUR PROOF OF CLAIM DOES NOT HAVE SUPPORTING DOCUMENTATION, IT WILL BE AUTOMATICALLY DISALLOWED.

6. DO NOT INCLUDE SOCIAL SECURITY NUMBERS IN PROOFS OF CLAIM EVEN THOUGH THE FORM SEEMS TO REQUIRE IT.
