The criminal trial is scheduled for Tuesday, May 22, 2007 at 9:00 a.m., U.S. District Court, 801 Broadway, Nashville, Tennessee 37203.
1. Download the B10 Proof of Claim form (above).
2. The claim form MUST be filled out completely and MUST include any attachments needed to support your claim. DO NOT SUBMIT ORIGINAL backup up documentation to support your claim; it will not be returned to you.
4. If you wish to have a confirmed copy of your claim returned to you, please bring an extra for yourself, or if you are mailing it, include an extra copy with a self addressed, stamped envelope with enough postage to cover return mailing costs. The Clerk will file stamp the extra copy and return it to you.
